In consideration of the registrants being allowed to participate in any of the indoor soccer and/or basketball programs including, but not limited to league tournaments practices, camps, clinics, droppings and or other athletic sports programs and recreational activities (hereinafter the “Activities”) at Integrity Sports Arena the parent of guardian of the registrant, a minor acknowledge and agree to the following:

2. I hereby give my approval of his/her participation in the activities at Integrity Sport Arena.

3. I clarify that the birthdate of the registrant is correct in that he/she is physically fit to engage in the physical demanding activities at Integrity Sports Arena.

4. I recognize the possible risk of physical injury associated with the activities, including risk of serious injury, permanent disability or death. Not all risk are foreseeable. For myself spouse and child, I freely and  knowingly assume all risk, both unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my child's participation.

5. I, for myself, my spouse, my child and on behalf of mine/our heirs, assigns, personal representatives and next of kin, hereby release and hold harmless Integrity Sport Arena and its affiliated organizations, sponsors, members, officers, agents, employees, and associated personnel, whether paid or volunteer against all claims, demands, and causes of action relating to injury, disability, death or any other loss or harm to person or property resulting from my child's participation in the activities at Integrity Sports Arena weather arising from the negligence of the releases or otherwise to the fullest extent permitted by law.

I, for myself, my spouse, my child and on behalf of mine/ours heirs, assigns, personal representatives and next of kin, hereby agree to indemnify and hold harmless the above releases from all liabilities relating to my child's involvement in the activities, even if arising from the negligence of the releases, to the fullest extent permitted by law.

7. I hereby give consent for emergency medical care as prescribed by a duly licensed doctor of medicine or doctor of dentistry. This care may be given under weather conditions are necessary to preserve the registrants' life, limb, health, or well-being. 

8. I understand that I am solely responsible for any cost arising out of any bodily injury, property damage and medical evaluation and rescue expenses resulting from the registrants' participation in the activities at Integrity Sports Arena. 

9. I agree that the registrants and I will abide by the rules and policies of Integrity Sports Arena and their affiliated organizations or sponsors. 

10. I understand and agree that Integrity Sports Arena may from time to time engage in the taking of photographs, videos, tape recordings, or other recording device related to the registrants' participation in the activities. I hereby give permission for the photos or products of such recording device to be used by Integrity Sports Arena in various marketing materials without additional notice or compensation.

I represent that I am the parent or guardian of the registrant listed above. I certify that I have read the above acknowledgements and agreement and that I fully understand and what and I knowingly and voluntary agree to the terms thereof Parents or Legal Guardian's signature.
